60 yrs old female ,who is a farmer, came with chief complaint of SOB since today morning decreased urine output and fever since 3-5 days. 
History of present illness :
Patient was apparently asymptomatic 5 days back .patient complaints of fever with chills high grade on and off relieved with medication for which she went to local hospital followed by Chief complaints of loose stools 10-12 episodes day before yesterday mucoid in consistency not associated with pus not ass with blood non foul smelling. Went to local hospital relieved on medication. Today morning she complained of SOB and Decreased urine output. No chief complaints of chest pain palpitations syncopal attacks 
SOB grade 4 ,orthopnea ,PND
Pedal edema grade 1
NO chief complaints of constipation abdominal pain ,abdominal distension. K/C/O type 2 DM since 15 years on regular medication on unknown medication
NO h/o HTN,  Epilepsy, CVA, CAD ,TB ,Asthma.
Vitals
PR-110bpm
BP- 110/70mm  
CVS-s1s2+
RS-B/L fine crepitations in inspiration IAA ISA
Perabdomen:soft ,tenderness + in umbilical region
Provisional diagnosis :
PRERENAL AKI SECINDARY SEPSIS ?UTI
